Back to Life separates recording a story, sharing a summary and receiving an individual treatment plan. One does not automatically authorize the next.
01 / YOUR RECORD
Make room for the whole story.
The preview is built to organize a symptom history, activity goals, check-ins and saved session observations. These are self-reported records, not a diagnosis.
Health-record storage starts off. The operator’s policy must be released and you must explicitly accept the current notice before health records can be saved.
02 / YOUR SUMMARY
See exactly what you are sharing.
Choose identity and goal, symptom story, recent check-ins, recent sessions or the latest plan. Preview the selected records before downloading a readable PT visit summary.
The recent-history windows are explicit: up to 50 check-ins, 20 sessions and one latest plan. Your full account export is a separate action. A downloaded file is not sent to a clinic.
03 / YOUR CARE CONNECTION
A named recipient. A separate permission.
When a real care team and service coverage are configured, a handoff requires a selected recipient, current care state, exact preview and explicit confirmation.
Ongoing care access starts off. If separately requested and accepted, the clinician can read the stated current records and manage their own assignments. A summary alone grants no ongoing access.
04 / CLINICIAN-ASSIGNED CARE
A plan needs more than a questionnaire.
Therapeutic directions require approved demonstration versions, a complete approved protocol, individual assessment and assignment, current permissions, and the session’s starting observations.
Patients cannot restart a paused plan by reopening a dialog or re-enabling storage. Related care must be reviewed and assigned again.
CHOICES THAT STAY SEPARATE
Storage. AI. Clinician sharing.
Each purpose has its own permission. AI also needs confirmation for each submitted question; saved symptom records are not attached automatically.
Withdrawing storage blocks future health writes, pauses care and revokes sharing. Your own records remain available for reading, export and deletion.
No. The symptom tools organize self-reported observations and next steps. They do not establish a diagnosis or choose a treatment direction.
Can I follow an exercise program today?
The clinical package is awaiting release. A questionnaire, video preview or saved report does not authorize exercise. Treatment requires the current approvals and an individual clinician assignment.
Will my health information be sent to AI?
AI sharing starts off. The operator policy must be released, your saved AI preference must be on, and you must choose to send each question. Saved symptom records are not attached automatically. Read the full data choices.
Does sharing a summary book an appointment?
No. A download stays on your device. A consented in-app handoff, when a real care team is configured, places the selected snapshot in that recipient’s inbox. Neither action sends an email or books an appointment.
