Information you choose to save

After you agree to the current notice, the app can save your account reference, profile details, symptom story, check-ins, goals, reminders, clinician assignments and treatment-session observations. Privacy choices and their versioned history are recorded even when health-record storage is off.

App records use the hosting service's database. Demonstration files use private object storage. Counts-only assistant and security request limits help protect the service from abuse, including when health-record storage is off. These counters do not store question text or symptom content. Assistant questions and answers are not saved as conversation records by this app.

A separate choice for AI questions

AI sharing starts off. A question is sent to OpenAI only when your saved AI-sharing preference is on for this notice and you also choose to send that question. The request contains that question and approved education passages; saved symptom records are not automatically attached.

The app requests store:false. This is not a promise of zero provider retention. Provider terms and data policies apply. Withdrawal does not recall a question already sent. Deterministic urgent-care guidance does not require AI sharing.

Sharing a clinician handoff

A handoff requires a separate explicit choice of recipient and the information to share. The app stores the selected snapshot and a record of the handoff. Ongoing access starts off and requires a separate active connection. Sharing, acknowledgement, decline, end-of-care and revocation status are recorded.

If you enable ongoing care, the connected clinician can read your current name, country, goal and assessment, the most recent 50 check-ins, and the latest 20 sessions and latest plan assigned by that clinician. This does not share reminders, privacy preferences, AI conversations or instructions assigned by other clinicians. A snapshot without ongoing care contains only the scopes you selected.

The app asks you to report your current care location to check the clinician's stated service area. It does not obtain GPS location. Confirm your current country and US state when asked before a session. A changed or unserved location pauses related care and requires a new care request, acceptance and assignment.

App authorization, directory entries and operator declarations do not independently verify a clinician's identity, license, availability or responsibility for your care. Confirm the recipient and arrangements directly. Revocation stops future app access; information already received, downloaded or copied cannot be recalled by this app.

Withdraw, export or delete

You can withdraw storage or AI-sharing permission in Privacy. Withdrawing storage stops future health-record writes, pauses app treatment plans and sessions, and revokes care connections. Existing records remain available for your own reading, export and deletion. Enabling storage later does not reactivate paused plans or revoked connections.

Deleting removes your patient, handoff and privacy-choice records from this app's active database. Exports on devices, copies already received by a clinician, provider systems and backups are not claimed to be erased by this action. Professional content-approval audit records are managed separately.

Operator details still needed

The service operator's privacy contact, retention policy and applicable legal arrangements have not been configured in this app. Obtain and review those details before using it with real patient information. This notice makes no HIPAA, regulatory-certification or legal-compliance claim.
